Children's Case Management

Children’s Case Management helps parents of children from birth to 21 years of age who have Intellectual Disabilities, Mental Health Disorders, Autism Spectrum Disorders, or other developmental delays/disabilities locate and access necessary services and supports for their children.

With the family, the Case Manager identifies the child’s strengths and needs, develops and coordinates a plan of care tailored to the child, connects families to services and supports, and helps ensure that the child’s needs are being met.
